GLP-1s and Pregnancy: What to Know Before Trying to Conceive
GLP-1s are not approved in pregnancy. Here's the washout timeline, fertility considerations, and a safe transition plan if you're planning to conceive.
The label guidance
Wegovy, Zepbound, Ozempic, and Mounjaro labels all advise stopping the medication at least 2 months before a planned pregnancy. The 2-month window reflects the long half-life of semaglutide and tirzepatide (about a week) plus a margin for full clearance.
There is no human data showing teratogenicity, but animal studies showed fetal harm at high doses — the precaution is conservative because better-controlled studies are not ethical to run.
Fertility effects (the unexpected upside)
Many patients with PCOS or insulin resistance see fertility improve on GLP-1s as weight drops and insulin sensitivity returns. Cycles regularize. This is also why the 'Ozempic baby' phenomenon is real — patients who weren't ovulating start ovulating, sometimes before they update contraception.
Planning the transition
- Tell your clinician you're planning to conceive 3–4 months out
- Taper rather than stopping cold — protects against rebound hunger
- Lock in protein, sleep, and movement during the washout
- Resume after delivery and breastfeeding (also contraindicated)
