In December 2024 the FDA approved Zepbound (tirzepatide) for moderate-to-severe obstructive sleep apnea in adults with obesity — the first medication ever approved for OSA. This expanded insurance coverage for many patients who previously couldn't access weight-loss GLP-1s.

Clinical considerations

An OSA diagnosis (typically AHI ≥ 15) plus obesity (BMI ≥ 30) is required for the on-label indication. A sleep study is needed for diagnosis if you don't already have one. Zepbound for OSA is NOT a CPAP replacement for severe cases — it should be considered alongside or as a step-down from CPAP under clinician supervision.
